Inspiration Home Health is looking for a full-time RN Case Manager. Inspiration is a fast growing home health and hospice agency offering a great work environment and excellent benefits!

Responsibilities:

• In home skilled nursing visits, initial and ongoing assessments
• Monitor and report changes in patient symptoms or behavior
• Communicate with collaborating physicians or specialists regarding patient care
• Maintain accurate patient medical records and plan of care
• Provide compassionate emotional support to patients and their family members
• Communicate effectively to interdisciplinary team members
• Observes/ensures compliance with Medicare and State guidelines

Qualifications:

• Previous experience in home health and hospice preferred
• Ability to build rapport with patients, families, and facilities
• Strong problem solving and critical thinking skills
• Ability to thrive in a fast-paced environment

Job Type: Full-time

Salary: $65,000.00 to $70,000.00 /year
